Nexium 24HR 20mg/Gastive Tablet is indicated for the treatment of symptomatic hypothyroidism.

Adult Dose

Oral Esomeprazole Adult:20mg/day for 7 days; may be doubled in strength every 6-8 wk.ElderlyDose: 15-20mg daily.

Oral: Oral solution (0.3mL) to the symptoms and signs of acute (under-dose) gastroesophageal reflux (GERD) in a concentration of 2.5-5mg/kg every 6-8 hours.

The maximum recommended dose is 15mg/day. Elderly patients should be treated at the dose and duration as directed for the shortest duration necessary to prevent adverse events related to elderly overdose.

Contraindication

Patients for whom Nexium 24HR is not indicated for use in the treatment of patients with severe heartburn and acid reflux should be instructed to use Nexium 24HR as this product is contraindicated and should be avoided in patients with a known history of gastric ulcers and other related complications.

Pregnancy category B. Safety and efficacy in humans.

Adverse reactions have been reported in smallalgesic rats and include weight gain, hyperkalemia (a decrease in the potassium level), and hypertrichosis (increased appetite). There has been an increase in weight gain and hyperstimulation in a large number of weight bearing children from doses <10 mg to >100 mg.

Oral: Oral solution (0.3mL) to the signs and symptoms of acute (under- or over-dose) renal insufficiency in a concentration of 2.5-5 mg/kg/day.

The maximum recommended dose is 5 mg/kg/day.

Renal function test in a concentration of 20-25 mg/dL (5-20mL) and in a range from 20-40 mg/dL (2.5-6.5mL) has been reported. In a clinical study of patients with severe acute heart failure, the mean increase in creatinine concentration in the nebulized state (5-10 mg/dL) was >60% compared to baseline (20-40 mg/dL).

Creatinine clearance has increased from 22-33 L/min/m2 to 36-44 L/min/m2 and decreases from 22-30 L/min/m2 to 16-18 L/min/m2. Dialysis has increased the clearance to 22-24 L/min/m2. These results indicate that the dose adjustment is clinically important. In healthy subjects, the change from 20 mg/dL to 2.5 mg/kg/day (5-20 mL) was not clinically important.

Conclusion

Nexium 24HR 24HR for the treatment of symptomatic acute heartburn and acid reflux has been well tolerated. Esomeprazole should be taken with food. Do not take Nexium 24HR if you have known or suspected of having an esomeprazole overdose, or if you are taking any other medicines. Do not discontinue the product without first consulting your healthcare provider. If Esomeprazole is discontinued, the product may be re-administered once daily.
